WHAT IS BHH?
A Behavioral Health Home is not a physical location, it is a team of professionals, working in tandem with your child’s primary care physician, dedicated to ensuring your child has the best possible care for their unique needs. The team includes:
- Clinical Team Leader
- Health Home Coordinator
- Youth or Family Support Specialist
- Nurse Care Manager
- Psychiatric Consultant
- Medical Consultant
HOW IT WORKS
Your child’s BHH team is a group of professionals dedicated to working together. This program is suited for children who benefit from comprehensive coordination of their physical and mental health care. The BHH team will communicate easily with each facet of your child’s health care team. Families should plan to see their care coordinator at least once a month. A BHH team can provide peer supports, coordinate health care services, provide education about your child’s diagnosis, develop treatment plans, complete referrals, advocate for educational accommodations, provide options for therapeutic services, help with goal setting, and more. BHH services include:
- comprehensive care management
- care coordination
- health promotion
- comprehensive transitional care
- individual and family support services
- referral to community and social support services
